  • Must successfully complete orientation and competency validation requirements.
  • Strong verbal and written communication skills with ability to effectively interact with all levels of management, internal departments and external agencies.
  • Working knowledge of various computer software applications.

Experience

  • Experience: Three years of RN
  • Must be detailed oriented, able to synthesize complex data and utilizes sound clinical judgement based on years of leadership and clinical experience.

  • The Clinical Coordinator is a vital member of the interdisciplinary healthcare team responsible for shepherding quality patient outcomes through evidence-based care coordination and frontline clinical respond and engagement.
  • This role focuses on early identification, response, treatment and documentation related to the high-risk diagnoses sepsis and stroke, while also participating in quality initiatives, education, and process improvement ideation.
  • Provides clinical oversight and direction to frontline staff in identifying and managing patients with the condition’s sepsis and stroke.
  • Monitors Emergency Department for signs of clinical deterioration and escalates concerns appropriately.
  • Responds to clinical alerts and assists with timely and accurate documentation related to patient status and care provided.
  • Educates staff on best practices, quality standards, and clinical protocols to enhance disease recognition, documentation, and management.
  • Contributes to quality improvement initiatives by identifying performance gaps, supporting interventions, and tracking progress.
  • Serves as a clinical resource across the enterprise for best practices related to disease-specific care and patient safety.
  • Provides direct treatment support during emergent patient scenarios as needed to ensure optimal outcomes.
