Responsibilities

Monitor therapy effectiveness and safety, identify optimization and intervene as appropriate.
Support quality‑improvement and REMS initiatives.

Hold a Doctor of Pharmacy (PharmD)
Must be a licensed pharmacist in Oregon
Candidate must meet one of the following [REQUIREMENTS] qualifications:
Completion of a PGY1 Residency and at least 1 year of ambulatory practice under a Collaborative Practice Agreement (CPA)

Salary: An employee in this position who performs the job duties in Oregon can expect a salary rate between $140,000 and $170,000 (Hourly Equivalent: Approximately $67.30 - $81.73 per hour, based on a 40-hour workweek.) plus bonus pursuant to the terms of an any bonus plan if applicable.
The actual salary will depend on experience, geographic location, and other factors permitted by law.
An employee in this position who performs the job duties in any other state can expect a salary rate between $127,000 and $180,000 (Hourly Equivalent: Approximately $61.06 - $85.54 per hour, based on a 40-hour workweek.) plus bonus pursuant to the terms of an any bonus plan if applicable.
