Responsibilities
- Monitor therapy effectiveness and safety, identify optimization and intervene as appropriate.
- Support quality‑improvement and REMS initiatives.
Requirements
- Hold a Doctor of Pharmacy (PharmD)
- Must be a licensed pharmacist in Oregon
- Certified as an HIV Specialist (AAHVIP)
- Candidate must meet one of the following [REQUIREMENTS] qualifications:
- Completion of a PGY1 Residency and at least 1 year of ambulatory practice under a Collaborative Practice Agreement (CPA)
Experience
- At least 3 years of ambulatory practice with disease state management
Benefits
- Salary: An employee in this position who performs the job duties in Oregon can expect a salary rate between $140,000 and $170,000 (Hourly Equivalent: Approximately $62.50 - $72.12 per hour, based on a 40-hour workweek.) plus bonus pursuant to the terms of an any bonus plan if applicable.

- We’re hiring an Ambulatory Clinical Pharmacy Specialist who will play a hands-on role in elevating patient care at a nationally recognized Academic Medical Center.
- In this dynamic, patient‑centered position, you’ll build meaningful relationships with patients and providers while delivering high‑quality medication management.
- You’ll work on‑site in clinic, collaborating directly with care teams and practicing under a Collaborative Practice Agreement (CPA) to conduct in-person and virtual patient assessments, order and monitor evidence-based treatment plans, optimize therapy, and resolve medication-related problems.

- Provide comprehensive medication management, including reconciliation, dose optimization, adherence support, and drug‑level assessments aligned with evidence‑based standards.
- Partner with physicians, specialists, and interdisciplinary teams to order individualized care plans under a CPA and promote seamless medication coordination.
- Provide recommendations to operations by supporting teammates as needed with complex prior authorizations, improving workflows, educating patients, and maintaining ongoing clinical development. What You Need:
