Responsibilities
- Facilitate referral to treatment, including linking patient back to PCP, to an organization integrated behavioral health provider or to a community partner for treatment Implement treatment plans, during and/or after course of treatment with integrated behavioral health provider Follow up with patients (by telephone or in person) to determine what variables such as depression, substance abuse, etc., may be affecting patient's ability to attend appointments and/or comply with treatment Work within and
- Manage patient registry lists and follow up with patients to monitor clinical outcomes and risk; consult with other providers regarding patient's return to care and/or possible changes in treatment plan Provides and participates in education presentations, case reviews and other department meetings as required or directed.
Requirements
- Qualifications: Undergraduate degree or equivalent
- experience Master's degree from and accredited school and active, unrestricted licensure as an LICSW or LMHC to practice independently in the Commonwealth of Massachusetts and 1+ years of
- experience in a Behavioral Health primary care integrated setting Preferred
- Candidates are required to pass a drug test before beginning employment.
