Coordinate care for long-term care residents using Ennoble Care’s EMR
Maintain regular, proactive, on-site engagement with residents to support care plans, follow-ups, and care transitions
Collaborate closely with Ennoble Care providers, within the long-term care setting
Accurately document patient interactions and care coordination activities
Requirements
Active, unrestricted clinical certification or licensure, such as:
experience in a long-term care, skilled nursing, or similar clinical setting
Strong working knowledge of healthcare terminology and care coordination principles
Proficiency with Microsoft 365, including Outlook, Word, and Excel
Ability to work full-time, Monday through Friday, 8:00 a.m. to 4:30 p.m.
Ability to pass a background check Full-time

Ennoble Care is seeking a full-time Care Coordinator, Embedded to join our team, providing on-site support within one or more partnered long-term care facilities in the Winchester, Virginia area. This role is an integral, in-person member of our clinical care team, working alongside Ennoble Care providers in a family-practice-style model of care that emphasizes continuity, relationship-building, and patient-centered support.
As an Embedded Care Coordinator, you are physically on-site in one or more partnered long-term care facilities, working alongside providers and facility staff.
You build consistent relationships with residents and families and become a trusted, familiar presence within the facility -- supporting care the way a traditional family practice would, but in the long-term care setting.
