Coordinate patient-centered care activities using Ennoble Care’s electronic medical record (EMR) system
Support Ennoble Care providers and clinical teams through accurate documentation, follow-up tracking, and care coordination
Collaborate closely with fellow Care Coordinators and internal care team members within an office-based environment
Maintain accurate, timely documentation in accordance with established workflows, documentation standards, and escalation protocols to support timely decision-making and efficient clinical operations
Cultivate and maintain positive, approachable working relationships while demonstrating a consistent, dependable presence
Requirements
Active, unrestricted clinical certification or licensure, such as:
experience in a clinical and administrative or care coordination role within a healthcare setting
Strong working knowledge of healthcare terminology and care coordination principles
Proficiency with Microsoft 365, including Outlook, Word, and Excel
Ability to work full-time, Monday through Friday, 8:30 a.m. to 5:00 p.m. EST, in an office-based setting in the College Park area of Atlanta, Georgia No weekends No on-call Daytime hours
Ability to pass a background check Full-time

Communicate patient needs, changes in condition, and urgent concerns to the clinical team in a timely and appropriate manner
Assist with care transitions, referrals, and coordination with external partners, as needed
