Qualified candidates must possess a minimum of a Medical Assistant (MA) certification with at least one-year of clinical experience, and be compassionate, detail-oriented professionals who thrive in a fast-paced environment.
Active, unrestricted clinical certification or licensure, in the state of Texas, such as:
experience in a long-term care, skilled nursing, or similar clinical setting
Strong working knowledge of healthcare terminology, diagnoses, medications, and care coordination principles
Proficiency with Microsoft 365, including Outlook, Word, and Excel
Ability to work full-time, Monday through Friday, 8:30 a.m. to 5:00 p.m. CST, (or thereabouts, contingent on facility hours) in an on-site role, based in the Waco and Temple, Texas areas

Ennoble Care is seeking a full-time Care Coordinator, Embedded to join our team, providing on-site support within one or more partnered long-term care facilities in the Waco and Temple, Texas areas. This role is an integral, in-person member of our clinical care team, working alongside Ennoble Care providers in a family-practice-style model of care that emphasizes continuity, relationship-building, and patient-centered support.
As an Embedded Care Coordinator, you are physically on-site in one or more partnered long-term care facilities, working alongside providers and facility staff.
You build consistent relationships with residents and families and become a trusted, familiar presence within the facility -- supporting care the way a traditional family practice would, but in the long-term care setting.

This Care Coordinator works directly within partnered facilities alongside providers, patients, families, and facility staff to support continuity of care, with a primary focus on Remote Patient Monitoring (RPM).
Responsibilities include supporting RPM enrollment and compliance, coordinating follow-up care, addressing barriers to care, communicating patient needs, and maintaining accurate and timely documentation within the EMR.
This role is very family-practice focused, highly relationship-driven, and centered around being proactive versus reactive when it comes to patient care.
